A simple matrix can connect each state to a check: access for accounts, URL for posts, notification for restrictions and measurement window for reach. The team can then route the incident to the right owner. This reduces back-and-forth and protects creators from being asked to remake content to solve a technical problem. The classification earns its value through a concrete next action, not through more elaborate terminology.

A content workflow can fail before publication, during execution, in audience reach, or after a viewer clicks. These failures can produce the same complaint in a meeting: “it did not work”.
